Social Services
Any District resident, regardless of income, may speak with a case manager during our walk-in hours. We provide a wide range of client-centered services and are flexible to the varying needs of the District’s vulnerable residents.
Intake
Monday-Thursday 9am-12pm and 1pm-4:30pm; Friday 9am-12pm.
Bread for the City’s agency-wide intake is a function of the Social Services Department. Each individual that comes to Bread for the City goes through a general intake. The intake process is a 15-30 minute interview that provides an introduction to our agency and services, as well as other federal and local programs that may be of assistance to our clients.
Walk- in and On-call Assistance
Northwest: Monday, Tuesday, Thursday: 9am-10:30am and 1pm-2:30pm; Friday 9am-10:30AM
CLOSED TO WALK-INS on WEDNESDAYS
Southeast: Monday through Thursday: 9:30am-11:30am
There is always a case manager available during the above times to provide walk-in and on-call assistance either in person or over the phone. During this time, case managers offer general information and referrals, helping clients identify and access a wide range of services available throughout the city.
Case Management
By appointment only
For clients needing a higher level of on-going support, we offer a case management program. This program is designed for clients that do not already have a case manager in the community and have ongoing complex needs and issues. Case management requires a commitment from the client to participate in regular meetings and work towards concrete goals. Individuals interested in case management should inquire during our walk-in hours in person or via phone. If appropriate, an appointment time will be scheduled for the client to meet with one of our social workers for an initial assessment.
Application Services
By appointment only
We provide assistance in helping clients to apply for public benefits including SSI/SSDI. Individuals interested in applying for Social Security Benefits should first consult with their doctor for feedback on if there is a medical or mental health condition that warrants an application for disability benefits. It is helpful to have the doctor complete a Medical Evaluation Report (MER) form. Interested clients can bring in that form during our walk-in hours to determine if we can assist with the application process.
If we feel we are able to assist in the process, an appointment will be scheduled with one of our social workers for an initial assessment, and the client may enter our case management program to reach their goal of receiving benefits.
We also have an SSI outreach program for individuals receiving case management services at another community agency that does not provide application services. In this case, the case manager should refer their clients to us for assistance.
If we are unable to assist with the application process, individuals can always apply on their own for Social Security benefits.
Representative Payee Program
To enter into our Representative Payee Program, consumers must be referred by their DMH Case Managers. A detailed description of this process can be found on the RPP pages.
